ALL Contractors must be entered into the Contractor Verification System (CVS), be verified and approved before they will be issued a CAC. Anyone whose information is entered incorrectly or not been verified will be denied a CAC and must reschedule their appointment after their information has been corrected.
IPAs, Non-DoD Government agents and Federal Affiliates who will require a Common Access Card MUST HAVE:
Completed (Typed) DD Form 1172-2 which has been validated by the proper agency official who has a signature card (DD FORM 577) on file at DARPA.
- All blocks must be completed and if overseas privileges are required, travel orders must accompany your 1172.
- Applicant will sign block# 37 in the presence of the Verifying Official that issues the CAC.
Please arrive 5-10 minutes prior to your appointment time. Due to the high volume of traffic at the CAC office, it is imperative that you arrive on time. Arriving late may result in cancellation of your appointment.
If you are unable to keep your appointment, please contact the CAC office at 571-218-4270 or via e-mail at CACAppointment@DARPA.mil to cancel.
- Two of the following forms of ID: current driver's license, current passport, social security card, voter's registration card, an active duty/dependent/retired military ID card, birth certificate, or a current CAC. (By current, we mean NOT EXPIRED) Company badges and credit cards are not accepted, only government issued identification.
- Your government (.mil or .gov) email address. (Personal email addresses; aol accounts, etc; will not be accepted)
- A six (6) to eight (8) digit number to use as a Personal Identification Number (PIN). It should not be a number derived from something easily known about you such as part of your SSN, birthday or anniversary date of you or a family member, telephone number, or address.
If you were born outside of the 50 United States of America, you MUST bring one of the following identification items with you for your CAC issue. Failure to bring this information will result in you not being issued a CAC.
- Birth Certificate (For US Citizens born abroad)
- Valid US Passport
- Naturalization Certificate
- US Government Document (normally issued by the Department of State) stating you are a US citizen
